Aged, Blind, and Disabled

Older adults and those living with disabilities must meet specific requirements to enroll in publicly funded health insurance. Our team works through these requirements to provide reliable enrollment services.

Overview

Medicaid Facilitated Enrollment for people above 65, or who are Blind, or Disabled.

Our Facilitated Enrollers are trained on  strict guidelines and procedures to provide you with assistance at no cost to:

  • Enroll into Medicaid (with or without Long-term Care), Medicare Savings Program, and Related Programs
  • Complete applications and renewals
  • Submit applications to New York City Human Resources Administration or the New State of Health Medicaid Eligibility Client Management system, and following-up on application status

We also provide assistance to apply to Medicare for people who need both Medicaid and Medicare (dual eligible).

Who We Serve

In order to be eligible for our Medicaid Facilitated Enrollment services, you must:

  • Be a resident of New York State
  • Be 65 year old or olderOr, be certified blind by the NYS Commission for the Blind (NYSCB)

    Or, living with disabilities, and certified Disabled by the Social Security Administration (SSA), State Medicaid Disability Review Team, or local (NYC only) Medicaid Disability Review TeamOr not Certified Disabled but Chronically Ill: An individual who is unable to perform at least two activities of daily living such as eating, toileting, transferring, bathing and dressing, or requires considerable supervision to protect from crisis relating to health and safety due to severe impairment concerning mind, or having a level of disability similar to that determined by the State Medicaid Disability Review Team.

If you are turning 65 within the next 3 months or you are 65 years of age or older, you may be entitled to additional medical benefits through the Medicare program.

You are required to apply for Medicare as a condition of eligibility for Medicaid. This is called Dual Eligibility, and we can help! If this applies to you, please call 1-800-517-6670.

How to Apply

It is best to give us a call to discuss in detail the document you will need to prepare your applications. Below are a few items you will likely need:

  • Proof home address dated within 6 months (lease letter, utility bill, postmarked envelope, driver’s license issued within last 6 months)
  • Proof of current income (paycheck stubs, letter from employer on company letterhead, social security award letter, unemployment benefits award letter)
  • Proof of your current insurance, if applicable (Medicare card, insurance policy)
  • Proof of current resources (bank account statements)

Medicaid and Medicare helps older adults and people with disabilities and chronic illnesses that interfere with their life need affordable care to continue to live fully and with dignity. Our Facilitated Enrollers are helping eligible New Yorkers apply and maintain their enrollment while complying with all the necessary requirements.
