Harlem Health Advocacy Partners (HHAP) bridges the gap between healthcare and social services for New York City Housing Authority (NYCHA) residents in East and Central Harlem.

Overview
Our Community Health Workers provide individual health coaching and group workshops at no costs, and build connections between residents, communities, healthcare providers, and service agencies in order promote health education, prevention and access to care.
Who We Serve
Our activities are open to NYCHA residents living in zip code 10029, 10035 or King Towers.
Program Assistance
Health Coaching
Community Health Workers provide residents with one-on-one coaching to manage chronic conditions, such as asthma, diabetes and high blood pressure. They provide residents with guidance on setting and meeting health goals, address health and wellness questions, facilitate communication with healthcare providers and connect residents to resources and social services. Health coaching is available to NYCHA residents living in zip codes 10029, 10035, or King Towers.
Educational Wellness Workshops
Community Health Workers conduct regular wellness activities (such as blood pressure monitoring and walking groups) and workshops (such as fitness classes, falls prevention, and stress management) with residents in NYCHA developments.
Community Outreach, Engagement, and Advocacy
Community Health Workers build trust and rapport with NYCHA residents, local organizations, and community leaders. They reach residents by organizing and participating in community health fairs, awareness campaigns, canvassing and door knocking, tabling, and other. The program engages residents in at least 3 advocacy campaigns per year, which are planned and run through a community organizing framework.
Navigating the Healthcare System
We partner with Single Stop’s Health Advocates to help residents find affordable or low-cost health insurance, read medical bills and review health plan options.
Impact
Since 2022, HHAP has provided thousands of Harlem NYCHA residents with one-on-one health coaching and referrals to community supports and hosted more than 700 community wellness events.Participants in the program show improvements in blood pressure and blood sugar control, mental health and general health.
