Our Certified Navigators are trained on strict guidelines and procedures to provide you with assistance at no cost with:

  • Applications to enroll in Medicaid, Essential Plan, Child Health Plus, and Qualified Health Plans
  • Comparing plan costs and benefits
  • Finding a plan that’s right for you
  • Updating and renewing applications

Who We Serve

We serve all residents of New York City and Nassau and Suffolk Counties. We discuss eligibility for Medicaid and other subsidized health plans.

How to Apply for Health Insurance

It is best to give us a call to discuss in detail the document you will need to prepare your applications. Below are a few items you will likely need:

  • Your (and your child(ren)’s, if applicable) social security number(s)
  • Citizenship/ Immigration Documents (U.S. passport, green card, work authorization card, naturalization certificate, letter from USCIS)
  • Employer and Income information for working household members (pay stubs, W-2 forms, recent tax returns)
  • Information about Additional Income (rental income, gambling winnings, etc.)
  • Award letters (unemployment, SSB, retirement, alimony)
  • Current Health Insurance Card(s) and Information for members of your household

We are able to work with you on your application submission over the phone or in-person at one of our locations.

Our Certified Navigators are trained to ensure that all eligible applicants are able to make informed decisions and enjoy a smooth enrollment process.

Every health plan in the Marketplace offers the same set of 10 Essential Health Benefits, including the following:

  1. Outpatient care — the kind you get without being admitted to a hospital
  2. Trips to the emergency room
  3. Treatment in the hospital for inpatient care
  4. Care before and after your baby is born
  5. Mental health and substance use disorder services. This includes behavioral health treatment, counseling, and psychotherapy.
  6. Your prescription drugs
  7. Services and devices to help you recover if you are injured, or have a disability or chronic condition. This includes physical and occupational therapy, speech-language pathology, psychiatric rehabilitation, and more.
  8. Your lab tests
  9. Preventive services including counseling, screenings, and vaccines to keep you healthy and care for managing a chronic disease.
  10. Pediatric services — This includes dental care and vision care for kids You can compare plans based on price, benefits, quality, and other features important to you before you make a choice.
